ABSTRACT

<p><b>OBJECTIVE</b>To explore the clinical effect of closed reduction and minimally invasive treatment of humeral shaft fractures in children.</p><p><b>METHODS</b>From July 2011 to April 2015, 39 cases of pediatric humeral shaft fractures were treated by closed reduction and external fixation, including 27 males and 12 females with a mean age of 8.6 years old ranging from 3 to 14 years old. Time from injury to the treatment was 2 h to 7 days with an average of 2.7 days. There were 6 cases of upper fracture, 21 cases of middle fracture and 12 cases of lower fracture. All children were closed injury, appeared pain, swelling, local deformity and limited mobility and other symptoms after injury. X-ray examination showed humeral shaft fracture. Neer score of shoulder joint function and HSS score of elbow joint function were used to record and analyze the pain, function and activity of shoulder and elbow joint before and after treatment.</p><p><b>RESULTS</b>All the 39 cases were followed up for 6 to 12 months with a mean of 8.6 months. Two cases appeared postoperative superficial infection of the needle, and healed after dress; other cases gained good pinhole healing. There were significant differences in the pain, function and activity of the shoulder of Neer score before and after the treatment (<0.05). There were significant differences in the pain and function of the elbow of HSS before and after treatment (<0.05). According to the evaluation of Neer score of shoulder function, the total score was 88.82±2.50, 29 cases were excellent, 9 cases were good, and 1 case was fair. According to the evaluation of HSS score of elbow joint function evaluation, the total score was 91.51±5.09, 30 cases were excellent, 7 cases were good, 2 cases were general.</p><p><b>CONCLUSIONS</b>Manual closed reduction combined with external fixation for the treatment of humeral shaft fractures in children has advantages of less trauma, definite reduction effect, reliable fixation and benefit for early functional exercise of the shoulder and elbow joint. This therapy can be used as one of clinical methods for the treatment of humeral shaft fractures in children.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effect of different manipulative reduction for children's distal radioulnar fracture and dorsal dislocation.</p><p><b>METHODS</b>From June 2013 to June 2014, 80 children with distal radioulnar fracture and dorsal dislocation were treated by bone setting manipulative reduction including 51 males and 29 females with an average age of 6.5 years old ranging from 3 to 14 years old. Time from injury to treatment was 1 h to 6 d, 31 cases were on the right, 49 cases were on the left. Among them, 45 cases were type I of overlapping displacement, 35 cases were type II. The displacement of the fracture was observed by clinical manifestations and X-ray examination. Under fluoroscopy, different techniques were used for reduction and fixation. After 3 weeks of over wrist fixation, the splints were overturned and fixed again for 1 to 2 weeks, then were removed. The wrist joint function was evaluated based on Dienst criteria.</p><p><b>RESULTS</b>Eighty cases of fracture were successfully operated one time, all reached anatomic reduction or near anatomic reduction. Eighty children were followed up for 3 months to 1 year. All the fractures healed, and the healing time was 4 to 5 weeks with an average of 4.6 weeks. All patients removed the splint 3 months later, the results were excellent in 72 cases, good in 7 cases and fair in 1 case, the excellent and good rate was 98.75%.</p><p><b>CONCLUSIONS</b>Bone setting manipulation for children's distal radioulnar fracture and dorsal dislocation can get good reduction. At 1 month after the removal of the splint, wrist function and finger strength gradually recovered and returned to normal after 3 months.</p>

ABSTRACT

PURPOSE: The purpose of this study was to identify novice nurses' experience of turnover. METHODS: Data were collected from June to August, 2014 through in-depth interview and E-mail. Twelve novice nurses with turnover experiences were selected through snowball sampling method. All interviews were recorded with MP3 recorder and transcribed for analyzing by Van Kaam method of phenomenology. RESULTS: The results were divided into 4 categories, 20 themes, and 119 formulated meaning related to turnover experience. These four categories were 'Excessively heavy working environment', 'Relationship problem', 'Physical/emotional withdrawal', and 'Realizing the absence of goal in job'. CONCLUSION: The result of this study will offer anticipatory information to understand of novice nurses' difficulties and to prevent their turnover. And these findings can be used for nursing administrators to design a turnover prevention program more effectively for nurses.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effect of Tongfu Jinghua Decoction (TJD) on hemodynamics and tissue oxygen metabolism in patients with post-traumatic sepsis shock.</p><p><b>METHODS</b>Totally 60 patients were randomly assigned to the treatment group and the control group, 30 in each group. Patients in the treatment group took TJD or were administered with TJD by nasal feeding in combined with conventional Western medical treatment, while patients in the control group only received conventional Western medical treatment. Changes of each index in hemodynamics and tissue oxygen metabolism were observed before treatment, and at 6, 12, 24, and 48 h after treatment.</p><p><b>RESULTS</b>Compared with before treatment in the same group, hemodynamic changes were significantly improved at each time point in the two groups. All indices of tissue oxygen metabolism at each time point of the two groups were significantly improved, except changes of O2 extraction ratio (ER) after treatment in the control group (P < 0.05, P < 0.01). Compared with the control group in the same period, heart rate (HR), systemic vascular resist- ance (SVR), and cardiac output (CO) were significantly improved with statistical difference (P < 0.05, P < 0.01). Mean arterial pressure (MAP), central venous pressure (CVP), and cardiac index (CI) were significantly improved at 6, 12, and 24 h after treatment with statistical difference (P < 0.05, P < 0.01). Each index of tissue oxygen metabolism in the treatment group were all improved at each time point with statistical difference (P < 0.05, P < 0.01).</p><p><b>CONCLUSION</b>TJD combined with conventional Western medical treatment could quickly improve hemodynamics and tissue oxygen metabolism disorder in patients with septic shock, and its curative effect was superior to that of conventional Western medical treatment alone.</p>

ABSTRACT

BACKGROUND: Aquaporin-4 may be one of the candidates for inducing the brain edema in ischemic stroke, however, it still has not been reported whether aquaporin-4 is involved in the formation of brain edema after intracerebral hemorrhage (ICH).OBJECTIVE: To observe the expression of aquaporin-4 of cerebral tissue in the pathologic course of hemorrhagic cerebral edema in rats, and investigate itsrelationship with the formation of brain edema following ICH.DESIGN: A randomized controlled experimental study.SETTING: Department of Neurology, People's Hospital of Guangxi Zhuang Autonomous Region; Research Room of Neurobiology, Chongqing University of Medical Sciences.MATERIALS: This experiment was carried out in the Research Room of Neurobiology, Chongqing University of Medical Sciences between April and October 2003. Totally 120 adult healthy male Wistar rats, weighing 250-300 g, were provided by the Experimental Animal Center of Chongqing University of Medical Sciences.METHODS: The experiment had four parts, each part (n=30) was divided into the control group (n=5) and operation group (n=25), and then the latter was subdivided into 6-hour, 1, 3, 5 and 7-day following ICH groups with 5rats in each group. In the operation group, ICH models were established by infusing collagenase into left caudate nucleus of rats unilaterally. The operative process in the control group was the same as that in the operation group except for infusing collagenase. Standards for successful model establishment: the paralytic forelimb flexed under abdomen after withdrawal,and the normal forelimb extended to the ground. ① The expression of aquaporin-4 was detected with immunohistochemistry. ② The expression of aquaporin-4 mRNA was assessed with in situ hybridizution. ③ The expression of aquaporin-4 mRNA was assessed with reverse transcription polymerase chain reaction (RT-PCR). ④ The pathological changes of hemorrhagic edema were observed under transmission electron microscope.MAIN OUTCOME MEASURES: ① expression of aquaporin-4 mRNAand protein; ② pathological changes of hemorrhagic edema.RESULTS: The model establishments were all successful in the operation group, and all the rats were involved in the analysis of results without deletion. ① Comparison of the expression of aquaporin-4 mRNA and protein among the groups. There was no significant change in the control group. As compared with the control group, there was a significant increase of the expressions of aquaporin-4 mRNA and protein in the edematous tissue at 6 hours following ICH, and then reached the peaks at 3 days, and the expression of aquaporin-4 was still significantly higher than that in the control group at one week following ICH (t=12.65, P < 0.01). ② The corresponding sequential pathological changes in the edematous tissue of rats in the operation group: There was a gradual increase of intracellular edema within 1-3 days following ICH, and then the brain edema became aggravated at 3 days, an emergence of vasogenic edema and local edema tissue degeneration and necrosis were observed.CONCLUSION: The increased expression of aquaporin-4 was obviously enhanced following ICH, suggesting that aquaporin-4 may play an important role in the pathological course of hemorrhagic brain edema, and inhibition of aquaporin-4 expression may be an effective pathway to prevent and treat brain edema.

ABSTRACT

<p><b>OBJECTIVE</b>To study the protection of puerarin on the cerebral injury in the rats with acute local ischemia.</p><p><b>METHOD</b>Rat was evaluated model of acute local cerebral ischemia was made by ligating middle cerebral artery. The cerebral damage toxylin and eosin((HE).</p><p><b>RESULT</b>The number of died neurons were significantly less in puerarin-treated rats than in the rats with cerebral ischemia (P < 0. 05). Similarly, the cerebral edema were significantly attenuated in the puerarin-treated rats as compared with cerebrally ischemic rats.</p><p><b>CONCLUSION</b>Puerarin can prevent the neuron from damage induced by acute cerebral ischemia.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of puerarin on the expression of Hsp (heat shock protein) 70 in the rats with cerebral injury induced by acute local ischemia.</p><p><b>METHOD</b>Rat model of acute local cerebral ischemia was made by ligating middle cerebral artery. The Hsp70 expression in brain tissue was detected by SP method of immunohistochemistry.</p><p><b>RESULT</b>The expression of Hsp70 was significantly higher in puerarintreated rats than those in the rats with cerebral ischemia.</p><p><b>CONCLUSION</b>Puerarin can enhance the level of Hsp70 expression in the rats with cerebral injury induced by acute local ischemia.</p>

ABSTRACT

BACKGROUND:It is crucial for us to find an objective and sensitive means for the assessment ofthe severitv and development of dementia,so as to make instant diagnosis and examination of therapeutic effect on patientswith vascular dementias(VD).Event related potential endogenetic eomponent P300 is proved closely correlated with the cognitive management of examinees but not with stimulus property,thereby considered as one of the most valuable and objective index for brain superior psychological activity.OBJECTIVE:To study the role and significance of events related potential P300,providing objective basis for clinical diagnosis and therapeutic assessment for patients with VD.DESIGN:Non-randomized simultaneous controlled explorative study based on patients. SETTING:Neurological department and electromyography department in aprovincial hospital and neurological department in a medical college hospital.PARTICIPANTS:Between February 2000 and June 2001,64 patients were of VD at neurological clinic and ward ofthe Second Affiliated Hospital of Chongqing Medical College,including 38 males and 26 females with theaverage age of(66.0±4.6)years(VD group).Culturallevel:19 cases re-ceiyed over senior school education,31 cases of junior school education and14 cases of illiteracy.Meanwhile 60 inpatients without VD were enrolled(non-VD group)including 35 males and 25 females withthe average age of(68.0±3.7)years.Culturallevel:senior school or above,15 cases,junior school, 29 cases and illiteracy,16 cases.METHODS:Mental sate of both VD and non-VD groups were assessed with mini-mental state examination(MMSE),and tested with P300 potential observing the variability of latency and amplitude of P300 in patients.Both methods were compared between VD group and non-VD group, moreover relationship between P300 latency and MMSE scores was also explored.MAIN OUTCOME MEASURES:Variability of P300 latency and amplitude Relationship between P300 latency and MMSE scores.RESULTS:P300 latency was found prolonged in 74% and 7% of VD group and non-VD group respectively,with obvious significance between them,meanwhile prolonged P300 latency was proved closely correlated with the MMSE scores(r=0.609 6,P<0.05).CONCLUSION:P300 latency can be considered as an obiective predictor for the earlier diagnosis and the assessment of therapeutic effects in patients with VD.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effect of Puerarin Injection on the hemorheology in acute blood-stasis model rats.</p><p><b>METHOD</b>The acute blood-stasis model rats were made by being soaked in ice water afer being injected adrenaline hydrochloride injection in a major dose. The changes of viscosity of whole blood and plasma, blood yield stress, erythrocyte aggregation and the maximum rate of platelet aggregation in the acute blood-stasis model rats were measured with Auto-Viscometer, and then the influence of Puerarin Injection on the hemorheology in the model rats was investigated.</p><p><b>RESULT</b>The viscosity of whole blood and plasma, and blood yield stress in the acute blood-stasis model rats were significantly higher than those in the normal control group (P < 0.01, P < 0.05). Both the high dose and the low dose of Puerarin Injection could reduce the viscosity of whole blood and plasma, blood yield stress and the maximum rate of platelet aggregation in the acute blood-stasis model rats (P < 0.01, P < 0.05). The high dose could also reduce the erythrocyte aggregation and the deformed Index of red blood cell (P < 0.05).</p><p><b>CONCLUSION</b>Puerarin Injection can ameliorate the hemorheology in acute blood-stasis model rats, and it has a dose-response relationship.</p>

ABSTRACT

AIM: To observe the ameliorative effect of Puerarin Injection on blood hyperviscosity in acute blood-stasis model rats. METHODS: The acute blood-stasis model rats were made by injected adrenaline repeatedly and soaked in ice water. The markers of whole blood viscosity and plasm viscosity in the rats were measured with Auto-Viscometer. RESULTS: The whole blood viscosity and plasm viscosity was significantly raised in the acute blood-stasis model rats and Puerarin Injection had ameliorative effects on them. CONCLUSION: Puerarin Injection can ameliorate the blood-high-viscosity in rats of blood-stasis model.

ABSTRACT

OBJECTIVE:To study the intervention mechanism of puerarin on expressions of HSP70 and Fas proteinum in rats with acute cerebral ischemia injury.METHODS:12 rats with acute cerebral ischemia injury were randomly divided into ischemia control group(saline) and puerarin treated control group,with 6 in each group,each rat under took own control by ischemia side(exp.group) and non-ischemia side(control group);And rats with acute cerebral ischemia injury were randomly divided into control group (saline,ischemia side),puerarin treated control group and normal group (non-ischemia side),with 6 in each group.And then the expressions of HSP70 and Fas proteinum is determined by HE staining and immunochistochemistry SP.RESULTS: The expression of HSP70 was negative or weakly positive in ischemia control group and puerarin treated control group, while that in ischemia experiment group and puerarin treated experiment group was significantly increased(P0.05);The numbers of dead cells in control group and puerarin group were found to be significantly larger than that in normal group(P
